At Shore Orthopaedic University Associates we understand the difficulties of not being able to work and provide for yourself and your family during times of injury. As a result, we provide services to assist in completing disability paperwork. We have a dedicated Disability Coordinator to complete your paperwork so that you may file it accurately and timely.
Unfortunately, this service is in large demand for a majority of our patients based on the type of complex injuries we often treat. Therefore, all forms require 7 – 10 business days for completion. We ask that you please be patient with the process.
To expedite completion of your form(s):
- Please fill out a copy of our Short Term Disability/FMLA Drop Off Form using the link below.
- Print and sign the Drop Off Form and submit it along with all of your disability forms and a $10 drop off fee (if applicable) to any of our locations.
*This fee WILL NOT be imposed for NJ State Disability Forms or Handicapped Placards.
